Okay so you should probably add a little more information to what exactly you’re requesting here, from what I can gather you want to know how to make your own ID? The website in your photo is a CAD/MDT system made with https://bubble.is/

Just take two minutes to gather what you’re actually trying to recieve from making a post, and making sure you’re putting it in the right place. Hope that solves your issue and if it doesn’t hopefully someone else will be able to.
